1. Fill out the appropriate application (either Home or Commercial) both can be found here
2. Bring a copy of the following
   1. Government issued ID of business owner
      1. Example: Driver's License
   2. If a commercial business, a copy of the building lease
3. Zoning of business location to be verified and approved
4. Building must be approved by City's building official
5. Occupational Tax fees paid
   1. $15 - Administrative Cost
   2. $.95 per $1000 estimated gross receipts

There are a few exceptions

• Restaurants
  • Require health inspection and food service permit issued by Carroll County Environmental Health Department
• Convenience stores
  • Require inspection and Department of Agriculture permit
• Health care, long term care and residential child care facilities
  • Inspected and regulated by Office of Regulatory Services (Georgia Department of Human Resources)
• Some professions are charged a professional fee instead of using the gross receipts method.
  • Insurance carriers
  • Lawyers
  • Physicians
  • etc.
